As a courtesy to you, we will be most happy to submit to your insurance plan. We are committed to helping you maximize your insurance benefits, and we will be happy to estimate your coverage. You will be asked to present a current insurance card at your first appointment. It is important that you provide us with current and accurate employer and insurance carrier information. For some plans, we may need the social security number of the insured member to submit claims or estimates.
Because insurance plans vary, and are vast in their complexity, we cannot guarantee coverage or benefits. Insurance plans that provide orthodontic coverage typically have a Lifetime Orthodontic Maximum which is separate from the Dental Maximum. Your insurance company gives us the estimated coverage but they will determine what will be paid after the initial claim is submitted.
In most cases you will need to remain effective under the plan throughout the completion of the treatment plan in order to receive the full benefit. Should you change plans or terminate coverage, you will be responsible to contact our office to provide the new information, and you will be responsible for any remaining insurance balance.
